How we handle your information. DTASA collects the details on this form to assess and administer your membership, arrange member insurance, publish your directory listing (if you choose to have one) and notify the Department for Infrastructure and Transport of your insurance status. Your date of birth is collected for insurance purposes only. The copy of your MDI licence is used once to verify your licence and is then permanently deleted. Your information is handled in line with the DTASA Privacy Policy.

By signing this form as a member of the Driver Trainers Association of SA (DTASA):

1. I will notify the DTASA immediately of any changes to my business or contact information.
2. I consent to DTASA providing my name and business contact number/s to the general public requesting Driving Instruction and/or assessment via the DTASA Office if specifically asked for.
3. I consent to allow the DTASA to contact me to notify of any industry updates or important information items.
4. All information provided on this form is true and correct and can be used when notifying relevant Government Insurance Brokers with regards to the current Privacy Policy.
5. I agree to comply with the Association's Constitution and Code of Conduct.
6. I understand that the DTASA will keep and store this information with regards to the current Privacy Policy.
7. I understand there is strictly no refund of any / all fees upon cessation.
8. I authorise the association to notify DIT on my behalf of my insurance status.
